  How to become a Client?

People wishing to become a client of our service can

  • Ring our office on 1300 138 794;

  • Be referred to our service by another group or organisation;

  • Be referred by an existing client, friend or relative.

Assessments

When you first ring the Service we will take your details and make arrangements for an assessment. If your need is URGENT please tell us and an assessment may be able to be provided immediately. The assessment usually takes 15 to 20 minutes on the phone. The reason for assessment is to determine whether or not you are eligible to use South West Community Transport services.

You will be asked for information such as

  • Name, address, phone number

  • Next of kin, local doctor

  • Medical condition

  • Disabilities, if any

  • Your transport needs

  • Level of assistance required during transport

You will also be asked for permissions to provide some of your information to government departments. This does not include your name and address. By providing such information, you will be assisting in the planning of future groups in your area. You have the right to refuse your information being given to a government department.

Should your assessment be approved you will be notified via post, and an information booklet will be forwarded within a month.

If you are assessed as not being eligible to use our services we will suggest other services which may be more appropriate and you will be advised under what circumstances you could reapply.
